When does this scene take place? Wasn't Miquella born long after the purge, when Shadow Keep and other places would have existed already? How did Torrent leave his company? Where is all the civilisation?

The bases of Miquella's footprints look exactly like Homeward (miracle) in DS1. I think this is FromSoft shorthand, hinting that Miquella yearned & planned to become a god all his life, as if that was his true home. True or not, I adore this symbolism.
Miquella really did abandon his "arm sinistral" (left), as the Belurat footprint says, which I presume is a metaphor for his desire to do any evil (sinister) to any being. It's literally gone.
